What problem is the innovation trying to solve?

"BC has the most rapidly aging communities in Canada. Patients living with chronic and episodic conditions are heavy users of acute care. Remote health monitoring has been proven to reduce preventable acute visits and hospital stays through early intervention and monitoring for patients from the comfort of their homes."

— Natasha Thambirajah, Director, Virtual Care Strategy, British Columbia’s Ministry of Health

The innovation:

"Our Home Health Monitoring (HHM) Program uses virtual care technology to enable remote monitoring of patients who provide health information from the comfort of their homes. HHM is a leading and transformative example of virtual care that enables more community, home-based and integrated care when and where patients need it."

— Natasha Thambirajah, Director, Virtual Care Strategy, British Columbia’s Ministry of Health
