Consumer Experience Platform

PwC’s Consumer Experience Platform is a mobile application that equips consumers with personalized, real-time prevention, care management and social reinforcement to better manage chronic conditions and improve health outcomes.

On the app, consumers participate in a health status assessment that formulates condition-specific educational content and convenient community resources. The app captures key data metrics including weight, blood pressure, glucose, A1C, and lipid panel and allows consumers to track, communicate and request follow-ups from care navigators.
