Collaboration fosters connected health

The system of caring for people — the chronically ill, the elderly or even the fitness buff — is reactive, expensive and cumbersome. But a new paradigm of care, called connected health, promises to reduce costs and improve quality by working with patients proactively. How? Patients monitor their own health using electronic devices outside of the provider setting. As a result, the number of visits to both physicians' offices and inpatient hospital units can drop significantly.
